Employee Form 1095-B Notice
Eligible former City employees may receive a copy of their 2020 form 1095-B upon request. To request a form 1095-B email the City Clerk. You must include an email address and a physical address to which the form may be sent. You can also call City Hall at 248-541-2901 with any questions.
The City will provide you with a form 1095-B within 30 days of the date the request is received.
